Job summary

Powdersville Post Acute, part of PACS Group, is seeking a compassionate Occupational Therapist (OTR) to join our rehabilitation team on a PRN basis in Easley, SC. You will evaluate residents, develop individualized plans, and collaborate with an interdisciplinary team to maximize independence and quality of life.

Responsibilities include documenting evaluations and progress, communicating with physicians and families, and ensuring compliance with state regulations.

Qualifications

• Bachelor's degree in Occupational Therapy required.
• Master's or Doctorate degree in Occupational Therapy preferred.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.
• Experience with Casamba and PointClickCare (PCC) preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide occupational therapy services per physician orders and established goals.
• Document evaluations, treatment sessions, progress notes, and discharge summaries accurately and timely.
• Monitor resident progress and modify treatment plans as appropriate.
• Communicate with physicians, nursing staff, residents, families, and other healthcare professionals regarding treatment plans and outcomes.
• Participate in care conferences, interdisciplinary meetings, and discharge planning.
• Educate residents, families, and caregivers on therapy goals and home programs.
• Conduct in-service training and education for facility staff as needed.
• Ensure compliance with state practice acts, facility policies, and regulatory requirements.
• Support residents in achieving their highest level of function, independence, and quality of life.
