Job summary

21st Century Home Health Services is seeking an Occupational Therapist to provide skilled in-home services across Contra Costa County (Pittsburg/Antioch). You will evaluate home safety, mobility, and daily living needs, and coordinate with the interdisciplinary team to keep patients safely at home.

Requirements include CA OT license and BLS/CPR; drivers license; flexible territory coverage; competitive salary with bonus potential and comprehensive benefits.

Qualifications

• Licensed CA Occupational Therapist required and in good standing.
• Active BLS/CPR certification preferred or required.
• Valid CA Driver's License and reliable means of transportation.

Responsibilities

• Provide skilled OT services in patients' homes to improve safety and independence.
• Assess home environment, falls risk, meds, seating, and mobility needs.
• Collaborate with Case Manager and care team for care plans.
• Educate patients and families, advocate for quality home care.
