Medical Providers Partners PLLC is seeking a dedicated Nurse Practitioner to join our clinical team. This role serves approximately 20 patients per day, offering autonomy and the opportunity to build strong patient relationships in a collaborative environment.

Ideal candidates are early- to mid-career clinicians with PA license, who thrive in a team setting and are committed to high-quality care, with on-call rotation and a focus on comprehensive, compassionate service.

Qualifications

• Active Pennsylvania Nurse Practitioner license and required certifications
• Graduation from an accredited Nurse Practitioner program
• Strong attention to detail and timely documentation
• Self-motivated, dependable, and patient-centered
• Willingness to participate in a rotating on-call schedule

Responsibilities

• Provide assessment, diagnosis, and management of acute and chronic conditions
• Conduct regular patient rounding and timely evaluations
• Order and interpret labs, diagnostics, and imaging
• Prescribe and manage medications in compliance with regulations
• Maintain accurate, timely documentation in the EMR
• Collaborate with nursing, rehab, pharmacy, hospice, and administrative teams
• Respond to change-in-condition and urgent care needs
• Support advance care planning and end-of-life discussions
• Participate in quality assurance and regulatory readiness efforts
