Curana Health is seeking Nurse Practitioners or Physician Assistants to deliver proactive, relationship-based primary care directly to seniors in communities across the region. You will diagnose and treat a range of conditions and manage care plans with support from physicians and operational leaders.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations & current board certification in your field.
• Masters Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Must possess excellent written and oral communication skills.
• Must possess traits of punctuality, organizational skills, and ability to function within a team.

Responsibilities

• Deliver high-quality primary care to residents in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic conditions with focus on preventative care and long-term wellness.
• Provide ongoing care management to improve quality of life and independence.
• Build meaningful patient relationships through regular facility rounds and collaboration with care teams.
• Maintain accurate EMR documentation and ensure regulatory compliance.
