Qualifications

• Must hold a valid nurse practitioner or physician assistant license in state of operations.
• Masters Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Excellent written and oral communication skills.
• Ability to function within a team and be punctual and organized.

Responsibilities

• Deliver comprehensive health assessments and develop individualized treatment plans for residents in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic conditions with a focus on preventative care and long-term wellness.
• Provide ongoing care management to improve quality of life and independence.
• Build meaningful patient relationships with residents, families, and care teams.
• Collaborate with physicians, clinical staff, facility leadership, and specialists to coordinate care plans.
• Maintain accurate EMR documentation and ensure regulatory and organizational standards are met.
• Travel to and from assigned facilities as required.
