Qualifications

• Active NP/PA license in the state of operation and current board certification
• Masters degree in Nursing or Physician Assistant Studies
• Strong leadership, interpersonal, and problem-solving skills
• Excellent written and oral communication skills
• Punctual, highly organized, and able to work well in a team

Responsibilities

• Deliver high-quality primary care to residents in skilled nursing, assisted living, and independent living communities.
• Build meaningful patient relationships through proactive care and clear communication.
• Drive clinical excellence by improving outcomes and reducing avoidable hospitalizations.
• Collaborate across the care continuum with physicians, clinical staff, facility leadership, and families.
• Provide ongoing care management to enhance residents' quality of life and independence.
• Monitor health status and intervene early to prevent complications.
• Educate residents and caregivers on disease management and wellness strategies.
