Curana Health is seeking Nurse Practitioners or Physician Assistants to deliver proactive, relationship-based primary care to seniors in skilled nursing and other living communities. You’ll provide autonomous, high-quality care with support from physicians and a tech-driven care model.

Qualifications

• Valid NP or PA license in state of operation and current board certification.
• Master's degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills.
• Excellent written and oral communication skills.
• Punctuality, organizational skills, and ability to function within a team.

Responsibilities

• Deliver high-quality primary care and develop individualized treatment plans for residents.
• Diagnose, treat, and manage acute and chronic medical conditions with preventative focus.
• Provide ongoing care management that improves quality of life.
• Conduct facility rounds and build relationships with residents, families and care teams.
• Collaborate with physicians, clinical staff, leadership and families to coordinate care plans.
• Maintain accurate EMR documentation and ensure regulatory compliance.
