Job summary

NHS is seeking an experienced Nurse Practitioner to provide care across the Practice, Enhanced Access Service and Primary Care Extended Access service. The post holder will assess, diagnose, treat and evaluate patients, working autonomously within professional boundaries and in collaboration with the multidisciplinary team.

The role covers six sites, a population of around 18,500, and requires meeting NMC advanced practice requirements, with leadership responsibilities and safe decision-making

Qualifications

• Registered nurse with NMC advanced practice requirements.

Responsibilities

• Provide autonomous clinical care, history taking, examination and management.
• Prescribe medications as per regulatory framework.
• Coordinate with multi-disciplinary team to plan and review patient care.
• Lead and support nurse-led services within the Practice and Extended Access.
• Maintain accurate records and adhere to Caldicott principles.
