Job summary

Ultra Health Providers (UHP) seeks a Nurse Practitioner to deliver comprehensive medical care onsite in skilled nursing and senior living settings. You will work autonomously within a collaborative interdisciplinary team to manage complex conditions and ensure continuity of care with residents and their families.

Qualifications

• Active, unrestricted Nurse Practitioner license in the applicable state(s) of practice.
• Current national Nurse Practitioner certification (ANCC, AANP, or equivalent).
• Experience in adult, family, and/or geriatric medicine is preferred.

Responsibilities

• Provide primary, urgent, and preventive medical care to residents in skilled nursing and, where applicable, assisted living settings.
• Independently evaluate, diagnose, and treat acute and chronic medical conditions.
• Conduct admissions and follow-up assessments, including exams, medication review, and screenings.
