Job summary

Ultra Health Providers (UHP) is seeking a Nurse Practitioner to deliver comprehensive medical care in skilled nursing and senior living settings. You will work with an interdisciplinary team to provide proactive, patient-centered care, manage complex conditions, and build trusted clinical relationships in a supportive environment.

Qualifications

• Active, unrestricted Nurse Practitioner license in the United States.
• Current national Nurse Practitioner certification (ANCC, AANP, or equivalent).
• Eligibility to work in the United States.

Responsibilities

• Provide primary, urgent, and preventive medical care to residents in skilled nursing and senior living settings.
• Independently evaluate, diagnose, and treat acute and chronic medical conditions.
• Conduct admissions and follow-up assessments, including exams and med reviews.
• Monitor resident condition and adjust treatment plans accordingly.
• Order and interpret diagnostics; prescribe medications within scope of practice.
• Collaborate with Medical Directors, attending physicians, and interdisciplinary teams.
• Communicate diagnoses and plans clearly with residents and families.
• Maintain accurate EMR documentation; ensure regulatory and payer standards.
• Participate in quality and care coordination to improve outcomes.
• Uphold HIPAA and regulatory compliance while maintaining ethical practice.
