Job summary

Ultra Health Providers (UHP) is seeking a Nurse Practitioner to deliver comprehensive medical care in skilled nursing and senior living environments. The role offers clinical autonomy, collaboration with an interdisciplinary team, and a strong focus on continuity of care and patient dignity.

You will serve as a consistent clinical presence, managing complex conditions, conducting admissions and follow-ups, and ensuring timely, evidence-based care for residents and their families.

Qualifications

• Active, unrestricted Nurse Practitioner license in the applicable state(s).
• Current national Nurse Practitioner certification (ANCC, AANP, or equivalent).
• Clinical training and experience in adult, family, and/or geriatric medicine.

Responsibilities

• Provide primary, urgent, and preventive medical care to residents in skilled nursing and, as applicable, assisted living settings.
• Independently evaluate, diagnose, and treat acute and chronic medical conditions.
• Conduct initial admissions and follow-up assessments, including physical exams and medication reviews.
• Monitor changes in resident condition and adjust treatment plans accordingly.
• Order and interpret diagnostic tests; prescribe medications and therapies within scope of practice.
• Collaborate with facility Medical Directors, attending physicians, nursing leadership, and interdisciplinary teams.
• Communicate diagnoses, treatment plans, and changes in condition clearly with residents and families.
• Maintain accurate and timely documentation in the EMR in compliance with regulations.
