Job summary

Medrina is a physician-owned group delivering high-quality rehabilitative care in post-acute settings. We expand in Gaffney, SC and seek an experienced Nurse Practitioner to join our SNF team, focusing on post-acute and rehabilitative care including orthopedic recovery, stroke management, and pain management.

You will work with a collaborative lead provider, enjoying a round-and-go model, strong mentorship, and autonomy.

Qualifications

• Active SC NP license.
• Active DEA license or ability to obtain prior to start.
• Minimum 1 year of NP clinical experience (SNF/geriatrics preferred).
• Experience managing adult/geriatric patient populations.
• Strong clinical decision-making and time management skills.
• Comfortable working independently in an SNF setting.

Responsibilities

• Provide comprehensive medical care to SNF patients, including assessments, diagnosis, and treatment planning.
• Manage patients with orthopedic rehab needs, post-stroke care, and chronic pain conditions.
• Ensure accurate and timely documentation in EMR.
