Job summary

As a Nurse Practitioner, you will provide comprehensive medical care, manage complex conditions, and build trusted clinical relationships with residents, supported by an interdisciplinary team.

Qualifications

• Active, unrestricted NP license in applicable state(s).
• Current national NP certification (ANCC, AANP, or equivalent).
• Eligibility to work in the United States.
• Experience in adult, family, or geriatric medicine.

Responsibilities

• Provide primary, urgent, and preventive medical care to residents in skilled nursing and, as applicable, assisted living settings.
• Independently evaluate, diagnose, and treat acute and chronic medical conditions.
• Monitor changes in resident condition and adjust treatment plans accordingly.
• Order and interpret diagnostic tests; prescribe medications and therapies within scope of practice.
• Collaborate with facility Medical Directors, attending physicians, nursing leadership, and interdisciplinary teams.
• Maintain accurate and timely documentation in the electronic medical record.
