Ultra Health Providers delivers onsite medical, mental, and behavioral health services within senior living environments. As a Nurse Practitioner, you will serve as a consistent clinical presence, delivering comprehensive care with autonomy while collaborating with an interdisciplinary team to improve residents’ outcomes.

You will manage acute and chronic conditions, perform admissions and follow-up assessments, and maintain thorough documentation for regulatory compliance.

Qualifikationen

• Active, unrestricted NP license in the applicable state(s).
• Current national NP certification (ANCC/AANP or equivalent).
• Clinical experience in adult, family, and/or geriatric medicine.
• Eligibility to work in the United States.

Aufgaben

• Provide primary, urgent, and preventive care to residents.
• Assess and manage acute and chronic conditions.
• Perform admissions and follow-up exams, including med review.
• Collaborate with facility leadership and interdisciplinary teams.
• Maintain HIPAA-compliant documentation.
