Job summary

Ultra Health Providers is seeking a Nurse Practitioner to deliver comprehensive medical care in skilled nursing and senior living settings. Qualifications

• Active, unrestricted Nurse Practitioner license in the applicable state(s)
• Current national Nurse Practitioner certification (ANCC, AANP, or equivalent)
• Clinical training and experience in adult, family, and/or geriatric medicine
• Eligibility to work in the United States

Responsibilities

• Provide primary, urgent, and preventive medical care to residents in skilled nursing and, as applicable, assisted living settings
• Independently evaluate, diagnose, and treat acute and chronic medical conditions
• Conduct admissions and follow-up assessments, including exams, medication reviews, and screenings
• Monitor changes in resident condition and adjust treatment plans accordingly
• Order and interpret diagnostic tests; prescribe medications within scope of practice
• Collaborate with facility Medical Directors, attending physicians, nursing leadership, and interdisciplinary teams
• Communicate diagnoses, treatment plans, and changes with residents and families
• Maintain accurate documentation in the EMR per regulatory and payer standards
• Participate in quality and care coordination efforts to support outcomes
• Adhere to HIPAA, state, and federal regulations while upholding ethical standards
