Job summary

Ultra Health Providers (UHP) seeks a Nurse Practitioner to deliver comprehensive medical care in skilled nursing and senior living settings. You will work independently within a collaborative team, managing complex conditions and building trusted clinical relationships.

This role emphasizes continuity of care, proactive management, and high-quality outcomes while maintaining regulatory compliance and patient dignity.

Qualifications

• Active, unrestricted Nurse Practitioner license in the applicable state(s).
• Current national Nurse Practitioner certification (ANCC, AANP, or equivalent).
• Clinical training and experience in adult, family, and/or geriatric medicine.

Responsibilities

• Provide primary, urgent, and preventive medical care to residents in skilled nursing and, as applicable, assisted living settings.
• Independently evaluate, diagnose, and treat acute and chronic medical conditions.
• Conduct initial admissions and follow-up assessments, including physical exams, medication review, and clinically appropriate screenings.
• Monitor changes in resident condition and adjust treatment plans accordingly.
• Order and interpret diagnostic tests; prescribe medications and therapies within scope of practice.
• Collaborate with facility Medical Directors, attending physicians, nursing leadership, and interdisciplinary teams.
• Communicate clearly with residents and families regarding diagnoses and treatment plans.
• Maintain accurate documentation in the EMR per regulatory standards.
• Participate in quality and care coordination efforts to support outcomes.
• Adhere to HIPAA and regulations while upholding ethical standards.
