Job summary

Ultra Health Providers (UHP) seeks a Nurse Practitioner to deliver comprehensive medical care to residents in skilled nursing and senior living settings. You will work independently within a collaborative team, managing complex conditions while building trusted relationships with residents and facility staff.

Role emphasizes autonomy, patient-centered decision-making, and continuity of care, with support from a multidisciplinary team and commitment to high-quality outcomes.

Qualifications

• Active, unrestricted NP license in the applicable state.
• National NP certification (ANCC/AANP or equivalent).
• Clinical training/experience in adult, family, or geriatric medicine.
• Eligibility to work in the United States.

Responsibilities

• Provide primary, urgent, and preventive medical care to residents in skilled nursing and, as applicable, assisted living settings
• Independently evaluate, diagnose, and treat acute and chronic medical conditions
• Conduct initial admissions and follow-up assessments, including physical exams, medication review, and screenings
• Monitor changes in resident condition and adjust treatment plans accordingly
• Order and interpret diagnostic tests; prescribe medications and therapies within scope of practice
• Collaborate with facility Medical Directors, attending physicians, nursing leadership, and interdisciplinary teams
• Communicate clearly with residents and families regarding diagnoses and treatment plans
• Maintain accurate documentation in the EMR in line with regulatory and payer standards
• Participate in quality and care coordination efforts to support outcomes
• Adhere to HIPAA and regulatory/Ethical standards of practice

Education

Active, unrestricted Nurse Practitioner license in the applicable state

Current national Nurse Practitioner certification (ANCC, AANP, or equivalent)
