Job summary

Deer Oaks Behavioral Health — Long-Term Care Clinical Services invites a Licensed Marriage and Family Therapist to join our integrated clinical team onsite at one or more local long-term care facilities. You will provide therapy, conduct assessments, and support care planning for geriatric residents.

You will work with an interdisciplinary team, enjoy a predictable Monday–Friday schedule, and benefit from robust administrative and clinical support that lets you focus on patient care and outcomes.

Qualifications

• Active, unrestricted LMFT license.
• Ability to work in long-term care settings.
• Strong documentation and compliance abilities.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Support residents with depression, anxiety, adjustment to illness, grief, trauma, and substance use.
• Deliver behavioral management and brief, goal-oriented interventions.
• Coordinate with nursing, social services, rehabilitation and medical providers to improve resident outcomes.
• Maintain complete and timely documentation using the Geriatric Notes Tool (GNT).
• Build strong relationships with facility staff and leadership.
