Job summary

Deer Oaks Behavioral Health seeks a Licensed Marriage and Family Therapist to join our integrated clinical team in long-term care facilities. You will provide therapy, assessments, care planning, and behavioral health support onsite with autonomy and robust clinical backing.

Qualifications

• Active, unrestricted LMFT license.
• Valid driver’s license and reliable transportation.
• Ability to work independently while collaborating with an interdisciplinary team.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Collaborate with nursing, social services, rehabilitation, and medical providers to coordinate care.
• Maintain complete and timely documentation using our Geriatric Notes Tool (GNT).
