Job summary

Deer Oaks is seeking a Licensed Marriage and Family Therapist to join its integrated clinical team in one or more long-term care facilities within your local community. You will provide individual, family, and group therapy and conduct psychosocial assessments to support residents’ mental health and adaptation to illness.

You will collaborate with interdisciplinary colleagues, develop care plans, and ensure compliant, timely documentation in a geriatric-focused setting.

Qualifications

• Active, unrestricted LMFT license required.
• Strong communication skills (verbal and written).
• Ability to work independently within an interdisciplinary team.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments and treatment planning.
• Collaborate with nursing, social services and medical staff to optimize resident care.
• Maintain documentation in the GNT system and ensure HIPAA compliance.
