Job summary

Deer Oaks Behavioral Health – Long-Term Care Clinical Services is seeking a licensed LMFT to join an integrated clinical team onsite at LTC facilities in your local community. The role focuses on therapy, assessments, care planning, and behavioral health support, with autonomy and robust administrative backing to prioritize patient well‑being.

You will work with interdisciplinary teams, perform clinical evaluations, and participate in care planning, while benefiting from ASWB‑accredited CEUs,

Qualifications

• Active, unrestricted LMFT license.
• Strong written, verbal, and interpersonal communication skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Commitment to timely documentation and high‑quality care.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Support residents with depression, anxiety, adjustment to illness, grief, trauma, substance use, and behavioral disturbances.
• Deliver behavioral management and brief, goal-oriented interventions.
• Coordinate with nursing, social services, rehabilitation, and medical providers to improve outcomes.
• Participate in care plan discussions, facility meetings, and case collaborations.
• Maintain documentation with our GNT and follow HIPAA guidelines.
• Build strong relationships with facility staff and leadership.
