Job summary

Deer Oaks Behavioral Health — Long-Term Care Clinical Services is seeking a Licensed Marriage and Family Therapist to join an integrated clinical team onsite in local LTC facilities. You will provide therapy, assessments, care planning, and behavioral health support, with a predictable Monday–Friday schedule and robust clinical backing.

Qualifications

• Active, unrestricted LMFT license is required.
• Valid driver’s license and reliable transportation.
• Strong written, verbal, and interpersonal communication skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Commitment to timely documentation and high-quality care.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, and treatment planning.
• Support residents with depression, anxiety, grief, and other LTC-typical conditions.
• Deliver behavioral management and brief, goal-oriented interventions.
• Coordinate with nursing, social services, and medical teams to optimize resident outcomes.
• Maintain complete documentation using the GNT and ensure HIPAA-compliant care.
