Blue Ridge Care is seeking an in-home Registered Nurse to join a PACE-based interdisciplinary team in Virginia. You will deliver direct clinical care in participants\' homes and community settings, prioritizing safety, independence, and dignity.

This role emphasizes autonomy, strong assessment skills, and meaningful relationships with participants and families, with field-based duties and collaboration with the IDT.

Qualifications

• Current Virginia RN license and BLS/CPR certification.
• Minimum of 1 year experience with frail or elderly populations.
• Strong clinical assessment, critical thinking, and independent work skills.
• Valid driver's license, insurance, and reliable transportation.

Responsibilities

• Perform in-home nursing assessments and ongoing participant evaluations.
• Deliver skilled visits including med administration, wound care, injections, and phlebotomy.
• Update care plans with the interdisciplinary team and educate participants/caregivers.
• Coordinate transitions after hospitalizations or ER visits and ensure safety at home.
