Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to deliver one-on-one geriatric care in patients' homes across Three Rivers, MI, including private residences and senior living communities.

Qualifications

• Active Physical Therapy license in MI or eligibility
• Experience in geriatric home-based care preferred
• CPR certification and patient safety focus

Responsibilities

• Provide one-on-one physical therapy to older adults in private residences or senior living communities
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support mobility, independence, and longevity
• Document effectively within FOX’s EMR system in accordance with regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth / virtual care when appropriate to extend access and flexibility
