Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to deliver one-on-one geriatric care in patients' homes and senior living communities in Wisconsin. You will evaluate and treat with evidence-based plans, document in the EMR, and collaborate with an interdisciplinary team.

Qualifications

• Valid Physical Therapy license in the state(s) of practice or eligibility to obtain.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in home or community settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support mobility, independence, and safety.
• Document in FOX's EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients and caregivers to promote aging in place.
