Job summary

FOX Rehabilitation is expanding in Grand Rapids, MI, seeking licensed Physical Therapists to deliver one-on-one care to older adults in private residences and senior living communities. Therapists will develop personalized plans, perform evaluations, and use telehealth to extend access when appropriate.

Experts in geriatric home care, FOX emphasizes autonomous scheduling, patient relationships, and meaningful outcomes in a flexible, home-based practice.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support mobility, independence and function.
• Document effectively within FOX’s EMR system following regulatory standards.
• Collaborate with an interdisciplinary team to coordinate care.
