Job summary

FOX Rehabilitation, Grand Rapids, MI, is seeking licensed Physical Therapists to provide one-on-one care to older adults in private homes or senior living communities. You will evaluate, plan, and deliver evidence-based therapy and document in FOX’s EMR system.

Qualifications

• Licensed Physical Therapist in the state(s) of practice.
• Degree from an accredited Physical Therapy program.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to promote mobility, independence and function.
• Document accurately in FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients and caregivers to promote safety and aging in place.
