Job summary

FOX Rehabilitation in West Bend, WI, seeks licensed Physical Therapists to deliver one-on-one geriatric care in homes or senior living communities. You can tailor your own schedule, work across private residences and communities, and optionally incorporate telehealth for greater flexibility and continuity of care.

Responsibilities include evaluations, personalized plans, and documentation in FOXs EMR. New graduates welcome through mentorship; H-1B sponsorship available for qualified clinicians.

Qualifications

• Valid Physical Therapy license in the state(s) of practice.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or communities.
• Perform evaluations and develop individualized care plans.
• Deliver therapy to improve mobility, independence, and function.
• Document sessions in FOX EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients and caregivers to promote safety and aging in place.
• Offer telehealth or virtual care when appropriate.
