Job summary

Atlantic Visiting Nurse seeks a compassionate Hospice Registered Nurse to join the interdisciplinary home care team. This role emphasizes autonomous clinical care, case management, and patient advocacy in patients' homes.

The RN will perform assessments, develop and adjust care plans, educate families, and ensure regulatory compliance while collaborating with physicians and other team members. This is a field-based, patient-centered position with a focus on quality of life.

Qualifications

• Active RN license required; Hospice or palliative care experience preferred.
• Ability to perform comprehensive nursing assessments and develop individualized plans of care.
• Strong documentation; knowledge of Medicare guidelines and hospice regulations.

Responsibilities

• Perform comprehensive nursing assessments to evaluate physical, emotional, and psychosocial needs.
• Coordinate care with physicians, social workers, hospice aides, chaplains, and IDT members.
• Educate patients and caregivers about disease progression, medications, and hospice philosophy.
• Document admissions, visits, and case management activities in the EMR as per agency standards.
