Job summary

Interim HealthCare is seeking a Hospice RN Case Manager to provide skilled nursing care in patients’ homes with autonomy and meaningful patient relationships. You will oversee a team, educate families, and help manage care plans while balancing a healthy work-life dynamic.

Qualifications

• Active, unrestricted RN license in Michigan
• At least 1 year of clinical RN experience (Hospice preferred)
• Current CPR certification
• Reliable transportation with valid license and insurance

Responsibilities

• Provide home-based care to patients in their homes
• Oversee LPNs, CNAs and aides providing patient care
• Assess patients, document progress and report changes to physician
• Educate patients on plan of care, medications, therapy, diet and exercise
• Coach family members on plan of care and home safety
• Coordinate discharge from services
