Capstone Hospice is seeking a dedicated RN Case Manager to deliver compassionate end-of-life care in the Smyrna/Marietta area. You will assess patients, develop care plans, manage pain and symptoms, and educate families, while coordinating with the IDG and in-office meetings on a regular schedule.

The role requires a Georgia RN license, accredited nursing education, and at least two years of nursing experience with direct patient contact in the recent past.

Qualifications

• Graduate from an accredited school of nursing.
• Currently licensed as a registered nurse (RN) to practice in Georgia.
• Minimum of 2 years as a registered nurse.
• Active patient contact within the past three years is preferred.

Responsabilités

• Observe signs and symptoms and report changes to physician and IDG.
• Teach, supervise and counsel patient and family about care.
• Organize work schedule and attend required meetings.
• Develop and re-evaluate the patient/family care plan with IDG.
