Job summary

Touchstone Health is seeking an experienced Registered Nurse to join our hospice team in San Antonio, TX. This highly autonomous position operates on a 7-on / 7-off schedule (Friday to Friday, 5:00 PM to 8:00 AM and every other weekend days/nights), offering meaningful time off and patient-focused care.

You will deliver total patient care, perform evaluations, revise plans, and coordinate services while supporting families and adhering to agency policies.

Qualifications

• Graduate of an approved professional nursing program with a current RN license.
• Ability to exercise initiative and independent judgment in patient care.
• Two years of nursing experience preferred.

Responsibilities

• Deliver nursing services per plan of care.
• Perform initial evaluations and regular reassessments.
• Initiate and revise care plan based on patient condition.
• Provide specialized nursing services requiring advanced skills.
• Coordinate services and communicate patient needs with team.
• Counsel patients and families about nursing care needs.
• Complete and submit OASIS assessments and care summaries.
• Take on-call duties for nights, weekends, and holidays.
