Job summary

The Care Team Hospice is seeking an experienced Registered Nurse (RN) to join our Cleveland-area team in a dedicated 7 On / 7 Off role. You will serve as a trusted clinical resource for hospice patients and families during evenings, nights, and weekends, providing symptom management and crisis support.

This role offers a unique schedule with 26 weeks off annually and the autonomy to make clinical decisions within an interdisciplinary team.

Qualifications

• Active RN license in Ohio.
• Minimum of one year nursing experience; hospice/home health/palliative care experience preferred.
• Strong assessment, symptom management, and critical-thinking skills.
• Ability to work independently and respond to changing patient needs.
• Excellent communication and compassionate approach to patients and families.
• Valid driver's license and current automobile insurance required.

Responsibilities

• Respond to after-hours patient and caregiver needs with timely nursing assessment and intervention.
• Conduct patient visits for symptom management and changes in condition.
• Educate patients and families on medications, symptom management, and end-of-life care.
