The Care Team Hospice in the Cleveland area is seeking an experienced Registered Nurse to join our team in a dedicated 7 On / 7 Off role. You will provide hospice care to patients and families during evenings, nights, and weekends, with seven days on followed by seven days off.

You will assess patients, manage symptoms, coordinate with the interdisciplinary team, and document in the EMR. Independence and sound clinical judgment are essential for compassionate end-of-life care.

Qualifications

• Active Ohio RN license.
• Minimum one year nursing experience; hospice/home health/palliative care preferred.
• Strong assessment, symptom management, and critical-thinking skills.
• Ability to work independently and adapt to after-hours schedule.

Responsabilités

• Respond to after-hours patient and caregiver needs, providing timely nursing assessment and intervention.
• Perform patient visits for symptom management and changes in condition.
• Provide education to patients and families regarding medications, symptom management, and end-of-life care.
• Coordinate with physicians, pharmacies, facilities, caregivers, and interdisciplinary team members to ensure continuity of care.
• Complete accurate and timely documentation in the EMR.
