Job summary

The Care Team Hospice is seeking an experienced Registered Nurse (RN) to join our Cleveland-area team in a dedicated 7 On / 7 Off role. You will provide hospice care during evenings, nights, and weekends, serving as a trusted clinical resource for patients and families and delivering symptom management, education, and crisis support.

This role offers autonomy in the field, strong interdisciplinary team backing, mileage reimbursement for patient visits, and comprehensive benefits including PTO,

Qualifications

• Active RN license in the state of Ohio.
• Minimum of one year nursing experience; hospice, home health, palliative care, oncology, emergency, or critical care preferred.
• Strong assessment, symptom management, and critical-thinking skills.
• Ability to work independently and confidently respond to changing patient needs.
• Excellent communication skills and a compassionate approach to patients and families.

Responsibilities

• Respond to after-hours patient and caregiver needs, providing timely nursing assessment, intervention, and support.
• Perform patient visits for symptom management, changes in condition, deaths, admissions, and other urgent clinical needs.
• Provide education and guidance to patients and families regarding medications, symptom management, and end-of-life care.
• Coordinate with physicians, pharmacies, facilities, caregivers, and interdisciplinary team members to ensure continuity of care.
• Complete accurate and timely documentation of assessments, interventions, and care coordination within the EMR.
