Residential Home Health and Residential Hospice is seeking a Hospice Certified Nursing Assistant (CNA) to provide in-home care in the Springfield, IL area. You will work 40 hours weekly in patients’ homes, delivering compassionate comfort-focused care and supporting families.

Requirements include an active CNA certificate, a high school diploma, and at least one year of healthcare experience. Travel to patient locations is expected.

Qualifications

• Active Certified Nursing Assistant (CNA) certificate in the state you work
• High School Diploma or certificate
• Minimum one year healthcare experience, Hospice experience preferred
• Valid driver’s license and ability to travel ~20% of workday to patient locations

Responsabilités

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide hands-on care to patients, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support comfort and quality of life
• Support and guide patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand what to expect during the hospice journey
• Respond promptly to patient needs with empathy and attentiveness and accurately document care in the EMR
• Work independently, but never alone
• Travel to patient homes within assigned territory
