Job summary

Corewell Health’s Visiting Nurses team is seeking an experienced RN for full-time home care in the Grand Rapids area. You will work with a multidisciplinary team to deliver skilled nursing services in patients’ homes, typically visiting multiple patients per shift.

The role offers autonomy in home health, a schedule including every 4th weekend and a defined 40-hour work week, with a sign-on bonus up to $10,000 for eligible licensed RNs.

Qualifications

• RN license in Michigan upon hire (STATE_MI)
• BLS certification from AHA or ARC (within 90 days)
• Driver's License – Michigan upon hire

Responsibilities

• Provide nursing services and treatments per the plan of care.
• Assess patient needs, monitor progress and communicate with the care team.
• Teach patients and families about care plans and medications.
• Lead and supervise CNAs, LPNs, and other staff as assigned.
• Ensure high-quality, cost-effective patient care and safety.
