Health Link is seeking a Home Health Registered Nurse (RN) Case Manager in San Mateo, CA. The role offers a $10,000 sign-on bonus and an hourly full-time schedule with a competitive on-target annual pay.

Responsibilities include managing caseloads, performing OASIS assessments, administering medications, wound care, IV therapies, and coordinating care with physicians and the interdisciplinary team while educating patients and families at home.

Qualifications

• Current, active California RN license (required). Please include license number with application.
• Minimum 1 year of professional nursing experience (required).
• Home health experience preferred; hospital and SNF nurses are encouraged to apply and we'll provide orientation/training.
• Valid California driver's license and reliable transportation (required).
• Strong clinical judgment, organization, and ability to work independently.

Responsibilities

• Manage caseload: Perform comprehensive OASIS assessments (Start of Care, Recertifications, Discharges).
• Provide skilled care: Administer medications, wound care (including wound vacs), and IV therapies.
• Coordinate care: Serve as primary contact for physicians and interdisciplinary team to keep the plan of care on track.
• Educate: Empower patients and families to manage conditions at home.
• Document: Complete accurate, timely clinical notes using AI-assisted tools.
