Qualifications

• Active Connecticut RN license required.
• Home health experience preferred, including OASIS.
• Hospital, med-surg, telemetry, LTC/SNF/rehab accepted.
• Strong clinical assessment skills.
• Valid driver’s license, reliable vehicle, and auto insurance.
• Excellent communication and documentation skills.

Responsibilities

• Provide skilled nursing visits and complete comprehensive assessments.
• Create, implement, and update individualized care plans.
• Deliver patient/caregiver teaching, medication management, and wound care.
• Communicate regularly with physicians, therapists, and the care team.
• Document all care accurately and on time.
• Maintain professional boundaries and promote patient safety at all times.
