Job summary

Advocate Health at Home is seeking a Home Health Physical Therapist to manage a patient caseload in-home across the North Side of Chicago, with a day schedule and rotating weekends/holidays. You will perform PT assessments, craft care plans, and collaborate with physicians to drive functional independence for patients.

This role requires IL licensure, at least one year of clinical experience, and proficiency with home-based therapy equipment.

Qualifications

• Licensed Physical Therapist in Illinois (or eligible).
• Minimum 1 year of clinical experience (or strong clinical affiliation).
• Experience with home-based therapy equipment and assistive devices.
• Strong communication, documentation, and time management skills.
• Ability to work independently while collaborating with a broader care team.

Responsibilities

• Provide comprehensive physical therapy assessments and develop individualized care plans.
• Deliver in-home therapy focused on improving mobility, safety, and independence.
• Serve as a case manager, coordinating care with physicians and interdisciplinary teams.
• Educate patients and families to support recovery and long-term outcomes.
• Travel throughout the assigned North Side Chicago territory.
