Advocate Health at Home is seeking a Home Health Physical Therapist to manage an independent caseload while collaborating with an integrated care team. You will assess mobility, design goals, and provide in-home therapy to enhance safety and independence for patients.

Qualifications

• Licensed Physical Therapist in Illinois (or eligible).
• Minimum 1 year of clinical experience or strong clinical affiliation.
• Experience with home-based therapy equipment and assistive devices.
• Strong communication, documentation, and time management skills.
• Ability to work independently while collaborating with a broader care team.

Responsibilities

• Provide comprehensive physical therapy assessments and develop individualized care plans.
• Deliver in-home therapy focused on improving mobility, safety, and independence.
• Serve as a case manager, coordinating care with physicians and interdisciplinary teams.
• Educate patients and families to support recovery and long-term outcomes.
