Elite Home Health is hiring a Physical Therapist (PT) to deliver skilled therapy services to patients in their homes around Leesburg and nearby communities. This PRN position requires a Florida PT license and CPR certification, with a goal of improving mobility and independence in patients’ daily lives.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Florida PT license in good standing.
• Minimum 1 year of PT experience preferred.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Perform physical therapy evaluations and develop individualized treatment plans.
• Provide skilled therapy including therapeutic exercises, gait training, balance training and mobility interventions.
• Educate patients and caregivers on exercises, safety, fall prevention, and assistive devices.
• Monitor patient progress and adjust treatment plans as needed.
• Complete timely and accurate clinical documentation.
• Communicate with physicians, nurses and the interdisciplinary care team.
• Supervise PTAs as required.
