Job summary

Elite Home Health is seeking a dedicated Physical Therapist to provide skilled in-home physical therapy across Florida. The PT evaluates mobility and function, develops and implements personalized treatment plans, and collaborates with the interdisciplinary team to improve safety and independence.

The role requires Florida PT licensure, active CPR certification, and at least one year of PT experience (home health preferred); a valid driver’s license is also needed to travel to patient homes

Qualifications

Graduate of an accredited Physical Therapy program.
Current state Physical Therapist (PT) license in good standing.
Minimum of one year of experience in physical therapy; home health, acute care, or rehab preferred.
Current CPR certification.
Valid driver’s license and reliable transportation.

Responsibilities

Evaluate mobility, strength, balance, endurance, and function; develop treatment plans.
Provide skilled PT in home health per physician orders and care plans.
Educate patients and families on goals, safety, and home modifications.
Complete timely documentation and ensure Medicare/state compliance; communicate with care team.
