We are hiring a Certified Nursing Assistant for hospice care in the Green Tree, PA area, providing hands-on support, ADL assistance, and compassionate care within an interdisciplinary team.

Applicants must hold an active CNA certificate in the state, have a high school diploma, and ideally Hospice experience.

Qualifications

• Active Certified Nursing Assistant (CNA) certificate in the state you work
• High School Diploma or certificate
• Minimum one year healthcare experience, Hospice experience preferred
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team
• Provide hands-on care to patients, assisting with activities of daily living (ADLs)
• Observe and report changes in condition to support comfort and quality of life
• Document care provided in the EMR
• Travel to patient homes within assigned territory
