Job summary

FOX Rehabilitation is expanding in Glen Allen, VA and seeks licensed Physical Therapists to deliver one-on-one care to older adults in private residences or senior living communities. You will assess, plan, and deliver evidence-based therapy to improve mobility and independence, with schedules you control and potential telehealth options.

Qualifications

• Must hold a valid Physical Therapy license in the state of practice or be eligible to obtain one.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency required.
• Current CPR certification required.

Responsibilities

• Provide one-on-one physical therapy to older adults in the home or community settings (private residences, senior living communities).
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that promote wellness, mobility and functional independence.
• Document effectively within FOX’s EMR system in compliance with regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers and senior living staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
