Job summary

FOX Rehabilitation is hiring licensed Physical Therapists to provide one-on-one care for older adults in New Castle, PA, either in private homes or senior living communities. The role supports independence, mobility, and quality of life using the Geriatric House Calls model.

You will have autonomy to schedule care, with opportunities to incorporate telehealth and other virtual care options to extend access and improve continuity of care.

Qualifications

• One-on-one physical therapy to older adults in home or senior living settings.
• Develop individualized plans of care and deliver evidence-based treatment.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists to support patient outcomes.

Responsibilities

• Provide one-on-one PT to older adults in homes or senior living communities.
• Perform evaluations and develop plans of care.
• Deliver therapy to improve mobility, independence and safety at home.
