Job summary

Cornerstones of Care in Kansas City, KS seeks an Aftercare Case Management Specialist to support children and families post‑reunification. The role requires autonomy, collaboration with families and the care team, and adherence to contract/licensing requirements to ensure safe, stable home environments.

The position focuses on coordinating services for aftercare and adoption cases, with emphasis on strong time management and sound decision making.

Qualifications

• Bachelor’s degree in Social Work or related field and 3+ years of work with children and families.
• Preferred LMSW/LSCSW or Licensed Professional Counselor with Master’s degree.
• Valid driver’s license and background checks required.

Responsibilities

• Coordinate and deliver aftercare case management services for children and families.
• Collaborate with the family, natural supports, and team to safely maintain children at home.
• Provide case management for adoption cases as required by contract/licensing/funding body.
• Demonstrate strong time management, judgment, and decision-making abilities.
