Job summary

BAYADA Home Health Care is seeking an Admissions Physical Therapist to perform Start of Care admissions and home health visits across Monmouth, Middlesex, Union, and Somerset Counties in NJ.

You will evaluate mobility, safety, and functional needs, then create individualized plans of care with physician orders for adult and geriatric clients. This role offers autonomy in a field-based setting with strong team support.

Qualifications

• Active New Jersey Physical Therapist license required.
• A PT degree from an accredited program is required.
• Minimum of one year of PT experience, with prior home health exposure preferred.
• OASIS experience is required as part of SOC admissions.

Responsibilities

• Perform OASIS Start of Care admissions visits for home health clients in the assigned NJ counties.
• Conduct comprehensive PT evaluations covering mobility, strength, balance, gait, transfers, and safety.
• Assess home environments for safety risks and independence barriers.
• Develop individual plans of care and establish PT goals with physician orders.
• Coordinate with nursing, social work, and other services to meet client needs.
• Educate clients and families on mobility, safety, and home health plan of care.
• Document evaluations and care coordination in the EMR.
• Support a smooth referral-to-admission transition and ongoing services.
