Job summary

BAYADA Home Health Care is seeking an Admissions Physical Therapist for full-time home health visits across Monmouth, Middlesex, Union, and Somerset Counties, NJ. The role focuses on admissions, SOC visits, and autonomous field-based work.

You will conduct PT evaluations, determine care plans, and coordinate with an interdisciplinary team to ensure safe, effective home health services. Prior home health and OASIS experience strongly preferred.

Qualifications

• Current and unrestricted New Jersey Physical Therapist license.
• Graduate of an accredited Physical Therapy program.
• Minimum of one year of recent experience as a Physical Therapist.
• Prior home health experience required.
• OASIS experience required.
• Strong clinical assessment, evaluation, and care coordination skills.

Responsibilities

• Complete comprehensive OASIS Start of Care admissions visits for home health clients in the assigned territory.
• Perform comprehensive physical therapy evaluations including mobility, strength, balance, gait, endurance, transfers, safety, and functional abilities.
• Assess home environment for safety concerns, fall risks, and barriers to independence.
• Develop individualized plans of care based on client needs, goals, and physician orders.
• Establish PT goals and make recommendations for ongoing services.
• Coordinate nursing, therapy, social work, and other home health services as needed.
• Educate clients and caregivers on mobility, safety, exercises, and plan of care.
• Collaborate with physicians and interdisciplinary team for coordinated care.
• Ensure timely completion of OASIS assessments and admission documentation.
• Document evaluations and care coordination in BAYADA EMR.

BAYADA Home Health Care is seeking an experienced Admissions Physical Therapist (PT) for a full-time opportunity with our UCV office, providing home health admissions and OASIS Start of Care (SOC) visits to adult and geriatric clients throughout Monmouth, Middlesex, Union, and Somerset Counties, NJ.

As an Admissions Physical Therapist, you'll play an important role in welcoming new clients to BAYADA and establishing the foundation for a successful home health experience. You'll complete comprehensive physical therapy evaluations, assess each client's functional and clinical needs, and develop individualized plans of care that help clients safely recover and maintain their independence at home.

How You’ll Make an Impact:

• Complete comprehensive OASIS Start of Care (SOC) admissions visits for home health clients throughout the assigned territory.
• Perform comprehensive physical therapy evaluations, including assessments of mobility, strength, balance, gait, endurance, transfers, safety, and functional abilities.
• Assess the client's home environment and identify potential safety concerns, fall risks, and barriers to independence.
• Develop individualized plans of care based on client needs, goals, functional abilities, and physician orders.
• Establish appropriate physical therapy goals and make recommendations for ongoing therapy services.
• Identify additional clinical needs and coordinate appropriate nursing, therapy, social work, and other home health services.
• Educate clients, families, and caregivers on mobility, safety, fall prevention, therapeutic exercises, and the home health plan of care.
• Collaborate with physicians and members of the interdisciplinary care team to coordinate comprehensive client care.
• Ensure timely and accurate completion of OASIS assessments and all required admission documentation.
• Accurately document evaluations, clinical findings, education, and care coordination using BAYADA's electronic medical record (EMR) system.
• Help ensure a smooth transition from referral to admission and ongoing home health services.

Qualifications:

• Current and unrestricted New Jersey Physical Therapist (PT) license.
• Graduate of an accredited Physical Therapy program.
• Minimum of one year of recent experience as a Physical Therapist.
• Prior home health experience required.
• OASIS experience required.
• Strong clinical assessment, evaluation, and care coordination skills.
• Excellent communication, interpersonal, organizational, and time-management skills.
• Ability to work independently and effectively manage a field-based schedule.
• Experience with electronic medical records (EMR) preferred.
• Reliable transportation and ability to travel throughout Monmouth, Middlesex, Union, and Somerset Counties.

Additional Information:

Base Pay, depending upon qualifications, status, and subject to negotiation:

$75.00 - $85.00 per point
