Job summary

Eventus WholeHealth seeks an Attending Physician to lead population health within the Outcomes-Centered Healthcare (OCH) model. You will collaborate with APCs to deliver high-quality, team-based care in client facilities, ensuring professional relationships and patient-centered outcomes.

This role may also serve in a Medical Directorship capacity, providing clinical leadership and oversight across facilities, with an onsite, Kentucky-based arrangement and a strong emphasis on physician

Qualifications

• MD/DO degree required; board certification in Internal Medicine, Family Medicine, or Geriatrics preferred.
• Active and unrestricted medical license in applicable state(s); DEA registration as required
• Minimum 2-3 years of clinical experience in post-acute, long-term care, or value-based care settings preferred
• Experience collaborating with interdisciplinary teams and facility leadership
• Familiarity with EMR systems (AthenaHealth preferred) and care coordination workflow

Responsibilities

• Lead population health efforts within Eventus WholeHealth’s Outcomes-Centered Healthcare (OCH) model
• Provide clinical leadership in client facilities and collaborate with Advanced Practice Clinicians toward excellence in patient care and professionalism in client relations
• Ensure high-quality, team-based care across assigned facilities
• Support Medical Directorship duties when designated, including clinical oversight
